This section shows the registration information currently on file for you with the College of Denturists of Ontario.
If your name has changed, you must inform the College in writing and provide appropriate documentation. Contact the College for more information.
If you have additional practice locations than can be indicated on the form, please also inform the College of these locations in writing. All practice locations must be reported to the College.
All members must report their hours of practice, which may be verified by the College for the:
2010-2011 registration year;
2009-2010 registration year; and
2008-2009 registration year.
All members report their continuing education hours for the:
2010-2011 registration year;
SECTION 2: CONTACT INFORMATION
This section shows the contact information currently on file for you with the College of Denturists of Ontario.
The registered address is the primary place of practice for Active members and the home address for Inactive members. The registered address is used by the College to determine the electoral district. All members must display at least one address on the College’s web-based Denturist Search where the public can contact them. For Active members, the registered address will automatically be displayed on the website. For Inactive members, they have the option to display their home address, an alternate address or a post office box.
The mailing address is the address that the College uses to contact you by mail. Unless you have indicated otherwise to the College, your registered address will be used as your mailing address. If you would like to use a different mailing address, please make corrections on the form. Normally, this address will not appear on the register or website, but you may choose to display it if you wish.
If you have alternate practice locations, you must report them to the College. You may choose whether or not they are displayed on the website.
If you are the owner of a denture clinic and have not had the name of the clinic approved by the Executive Committee of the College, please be aware that it is the responsibility of the denturist who owns a denture clinic to obtain the approval of the Executive Committee of the College to name the clinic anything other than the Registered name of the denturist owner. SECTION 3: MEMBER PHOTOGRAPH The College requires members keep registration information up-to-date. Please provide the College with a passport size current photo of yourself if you registered with the College before 2001.
SECTION 4: LANGUAGE INFORMATION
As part of the changes to information collected by the College as required by Bill 171, we will now be collecting information on member languages used.
- Preferred language will not appear on the register or website.
- Languages in which you are able to provide denturism services will appear on the register and the website. Members who list other languages should ensure that they are able to actually provide services in those languages in order to avoid complaints. SECTION 5: REPORTING REQUIREMENTS You must answer all of the questions as either yes or no. If you answer “no” to the first question, you must provide full details in the comment section at the end of this section. For all other questions, if you answer “yes” provide the details of the incident in the space provided at the end of this section. SECTION 6: INSURANCE Members registering with Active status are required to obtain professional liability insurance as required in the College by-laws. You must provide the College with proof of professional liability insurance no later than 4pm on April 15, 2011.
