FEES

Members registering with Active status are required to obtain professional liability insurance as required in the College by-laws. You must provide the College with proof of professional liability insurance no later than 4pm on April 15, 2011.

Fee Payment
All members are required to pay annual registration renewal fees as outlined below: 

Active                            $1426.42+$185.43 HST=$1611.85
Inactive                         $713.21+$92.72 HST= $805.93
Retired Life Member    
$145.71+$18.94 HST = $164.65
   
                      
·Members may choose to pay by credit card, email money transfer or by cheque. Cash is not acceptable.
·Fees received after 4pm on April 15, 2011 are subject to a 10% late payment penalty. 
·Additional penalties for non-payment of fees include suspension of certificate of registration.
·Please note: credit card payments are processed in real time.
·Payments declined and/or returned by the bank for any reason are subject to a further $45.00 service charge in addition to the late payment penalty.   

Installments: 
Members may choose to pay Active or Inactive registration fees either in one lump sum or two installments.  Retired Life Members do not have the option of installment payments.

Installments are subject to a $50.00 plus HST service charge for each installment. Should the initial installment payment be declined and/or returned by the bank, the full registration fee and penalties will be payable immediately.

Installment payments are due as follows:

Active fees
($713.21+$92.72 HST)+($50.00+$6.50 HST)=$862.43 due by April 15, 2011
($713.21+$92.72 HST)+($50.00+$6.50 HST)=$862.43 due by October 15, 2011

Inactive fees
($356.61+$46.36 HST)+($50.00+$6.50 HST)=$459.47 due by April 15, 2011

Members must provide the College with a credit card that is valid at the time of processing. Note that credit card payments are processed in real time. 
If the member chooses to pay by cheque, both cheques must be received at the College office by 4pm on April 15, 2011. Installment payments received without the 2nd installment cheque will be considered incomplete and as a late payment.  A late payment penalty fee of 10% will be applied to the full amount.
Void cheques will not be accepted for payment.
Payments declined and/or returned by the bank for any reason are subject to a further $45.00 service charge in addition to the late payment penalty.   


PAYMENT OPTIONS
Each member must choose ONE of the following payment options.

·Credit Card
Online check the type of credit card to be used and complete the credit card number and expiry date. Choose either the full or installment payment(s) option. Payment(s) will be processed on April 1, 2011 and as they are received following April 1, 2011. If paying in installments, the second payment will not be automatically processed on the credit card provided on October 15, 2011. The registrant must log back in to make the payment prior to 4pm on October 15, 2011. Credit card payments are processed in real time. If the card payment is declined by the bank, the registrant will be contacted by the College, and will be due a service charge and penalty.
Please note that all credit card payments will be subject to a $40 convenience fee.

·Cheque
Online check the appropriate box indicating full or installment payment(s). An email notification will be automatically sent to the College of your selection. Mail the cheque(s) to the College office by 4 pm April 15, 2011. When paying installments by cheque, post date the second cheque for October 15, 2011 . Both installment cheques must be mailed and received at the College office by 4 pm April 15, 2011. Installment payment received without the 2nd installment cheque will be considered incomplete and as a late payment. A late payment penalty fee of 10% will be applied.

The option of pre-authorized payment is not available
VOID CHEQUES WILL NOT BE ACCEPTED FOR PAYMENT
